CHRONICALLY ILL AND DESTITUTE

Who We Serve

We serve mainly low-income seniors across a wide spectrum of frailty
and ageing needs. These include personal care and caregiving
services to pre-frail and frail seniors living at home; those with
chronic diseases who need round-the-clock residential care; and
medical and nursing care for the home-bound terminally ill.

We also offer refuge to the homeless at our
home for the destitute.

OUR APPROACH

Our care addresses holistic needs by providing medical, nursing, rehabilitative, and home-based care, to services that support the familial, psychosocial and spiritual needs of our beneficiaries, their families and caregivers. Our purpose is to promote better health, care, and life, through the philosophy of active ageing.

Resident-directed

Normalisation is restored to the beneficiaries’ lives with targeted personalised care and appropriate autonomy for the level of care required, in a family home-like environment

Communal Belonging

Building a community by encouraging interactions and caring relationships between residents, in a family home-like environment, with dementia-friendly care amenities for intermediate-to-long-term stay

Holistic and Integrated

Multi-domain care across medical, nursing, and rehabilitative needs, with consideration for the beneficiaries’ total welfare

Daily Living Care

Shelter, meals, safe haven provided for the destitute and sick

Home-based Care

From personal care and housekeeping, caregiver training and support, to medical and nursing services and round-the-clock support for those with life-limiting conditions
